1. Nature of Services & Clinical Oversight
Elite Transformation operates as a multi-disciplinary wellness and medical optimization platform. Clinical medical services—including Testosterone Replacement Therapy (TRT), prescribed peptide protocols, and diagnostic laboratory evaluations—are performed exclusively by licensed, credentialed healthcare providers acting within their medical scope of practice. Non-clinical services, such as lifestyle architecture, habit recalibration, and nutritional coaching, are designed purely for educational and motivational support.
2. Scope of Medical vs. Coaching Protocols
Clinical Protocols: Medical treatments and prescriptions are intended to treat specific physiological deficiencies (such as clinical hypogonadism) and require an active provider-patient relationship, medical history review, and laboratory verification.
Wellness Coaching: Our lifestyle "Recalibration Protocols" and coaching sessions do not constitute medical diagnoses or prescriptions. They are intended to support general wellness, sleep hygiene, and healthy habits.
3. Client Responsibility
By engaging in our services, the Client acknowledges that they must provide accurate health histories to our medical staff. The Client agrees to follow all prescribed medication guidelines exactly as directed by their treating clinician and understands that lifestyle adjustments are undertaken at their own discretion.
4. Laboratory Management & Diagnostics
Elite Transformation acts as an administrative and clinical bridge between clients and accredited third-party diagnostic facilities. We manage the logistics, ordering, scheduling, and data presentation of lab work for the clinical evaluation of the patient by our healthcare providers.
5. Prescription Product Disclaimer
All prescription medications, hormone therapies, and therapeutic peptides dispensed or coordinated through Elite Transformation are sourced from regulated, licensed pharmacies. These treatments are fully intended for human therapeutic use as prescribed by our medical team. They have been manufactured and prepared in accordance with applicable federal and state compounding laws and are strictly for the individual use of the prescribed patient.
